By meeting the requirements
of the Well Credit Program, you may be able to get a credit for part of your
in-network deductible for the 2012 plan year. The exact credit may be modified
depending on the plan design in 2011. See the links below to learn how to earn your
credits in 2011 for a benefit in 2012. Follow the steps below to get going!
Your username is ccob + your 4 digit ID number +
the letter e (if you are an employee) or the letter s (if you are a
spouse). So for example, it would look like this:

Your password initially is wellness, unless you were hired or enrolled in the plan after 1/1/2010, then
your password is wellness1. If
you have ever logged on to the site,
you were prompted to change your password. If you have done
this, please use the password you created!

All credits must be earned between December 1,
2010 and November 30,
