Tips on Finding Your Provider in the Anthem HMO network:

 

Easiest - Call your doctor and ask if he/she is in the Anthem HMO network (We will be using the HMO network not the HMO Select network).  Ask him/her for his/her PCP ID number.  Write that number down (And don't forget to get PCP IDs and doctors' names for your enrolled dependents) as you'll need to send the doctors' names and PCP IDs it to HR once you receive the form from us.

 

Other Methods:

1) Go on-line to www.anthem.com and go into the site as a member and then click on "Find a Doctor".  Click on Colorado and then be sure to select "HMO".  Enter the criteria for your search and when the Doctor name(s) pull up, you should be able to click on the Doctor's name to find the PCP ID number.

 

2) If you have tried all of the above methods and you still can't find your doctor, call HR.  We'll check with Anthem and get back to you.  If you doctor is not in the HMO network, don't despair.  We'll ask Anthem to initiate a provider outreach to see if the doctor will join the network.
