Colorado Family Medicaid

Colorado Family Medicaid is a public health insurance program for Colorado children age 0-18 and for parents with dependent children.  The program is funded jointly by a federal-state partnership and is administered by the Department of Health Care Policy and Financing. 

To qualify, families or individuals must:

• Have a child, be pregnant or be 18 years of age or younger

• Live in Colorado

• Meet the Medicaid income limits

• Be a U.S. citizen, a legal permanent U.S. resident for at least 5 years, an asylee, a refugee or a non-citizen with a qualifying emergency

To apply:

Applications (in English or Spanish) may be obtained in the following ways:

• Online by visiting the Children's Health Plan Plus website at www.CHPplus.org or the Colorado Public Health Insurance for Families (CPHF) website at www.chcpf.state.co.us. 

• By visiting the Broomfield Health and Human Services Department at 6 Garden Center, Broomfield, CO 80020.

• By calling the Broomfield Health and Human Services Department at (720) 887-2287 or CPHF at 1-800-359-1991.

The application must be completed and submitted to the Broomfield Health and Human Services Department.

Cost:

Children and pregnant women do not have to pay any co-pays.  Other adults may have a small co-pay (what you pay each time you get a service).

Aged, Blind or Disabled Medicaid

Medicaid is available for persons over the age of 64, or for persons who are blind or who have disabilities. 

To qualify for Medicaid as an aged person, the person must:

Be over the age of 64 and eligible for Supplemental Security Income (SSI) and/or Old Age Pension (OAP) state supplemental payments.

• Apply for SSI if not already a participant.

• Not have monthly income exceeding $662.

• Not have assets exceeding $2,000 for an individual and $3,000 for a couple.

To qualify for Medicaid as a person who is blind or disabled and under the age of 64, the person must:

• Be determined blind or disabled by the Social Security Administration (SSA) standards.
  

• Apply for SSI if not already a participant.
  

Note: If the person is eligible for SSI from SSA, the person should automatically receive Medicaid; however, if you are on SSI and have not been enrolled in Medicaid, contact the Broomfield Health and Human Services Department.

Long Term Care Medicaid

Long Term Care Medicaid includes nursing facility care and Home and Community Based services where medical providers come into the home to render the necessary services to keep a disabled individual from being institutionalized in a nursing facility or hospital.

Disability Eligibility:

Individuals of any age requiring long term care services must meet the Social Security disability criteria either through Supplemental Security Income (SSI) eligibility or Social Security Disability Income (SSDI) eligibility.  Colorado also has a state contractor who can determine disability if an individual has not been determined disabled by the Social Security Administration.

Income Eligibility:

The income limit is $1,911 a month.  For those individuals who are not hospitalized, an income trust can be established if one's income exceeds $1,911 a month.  There are limits for the income trust which are dependent on the county in which the applicant resides.

Only individual applicant income is considered in the eligibility determination.  An applicant's spouse's income is not considered.

Resource Eligibility:

• The individual resource limit is $2,000.

• The couple resource limit is $4,000 if both members of a marital couple are applying for long term care benefits and they are sharing the same room in a nursing facility or if both are going to receive Home and Community Based Services.

• The couple resource limit is $3,000 if both members of a marital couple are applying for long term care benefits and are living in separate rooms in a nursing facility.

• The couple resource limit is $111,560 (for 2009) if one member of the couple is applying for long term care services and the other spouse is not applying for Medicaid and is not institutionalized.

Additional Eligibility Requirements:

• Applicants of long term care Medicaid must receive long term care services for 30 consecutive days before eligibility can be approved.

• Applicants of long term care Medicaid cannot transfer assets for less than fair market value within 3 years of the date of application.  If any transfers were made, the applicant may be ineligible for long term care services for a period of time.

Early and Periodic Screening Diagnosis and Treatment Programs (EPSDT)

EPSDT is the child health component of Medicaid.  It is required in every state and is designed to improve the health of low-income children by enhancing access and appropriate utilization of primary and preventive health care for Medicaid-eligible families and children.  EPSDT is designed to address physical, mental, and developmental needs.   EPSDT is available for pregnant women of all ages and children, ages newborn up to 21 years of age who are enrolled in Medicaid.  For more information on this program, please visit the EPSDT web page.
