Public Health and Environment Division Health Care Program for Children with Special Needs The Health Care Program for Children with Special Needs (HCP) provides several levels of services for any child, from birth to 21 years of age, that has been identified with special needs. The goal of the program is to support families in meeting their goals and priorities regarding their child's health and is designed to assist families in obtaining information, referrals and support. Services are supported through the Colorado Department of Public Health and Environment (CDPHE) and are provided at no cost to clients. Anyone may refer a child to HCP. HCP works with a regional team of specialists, including a nurse, speech pathologist, occupational/physical therapist, social worker, audiologist, nutritionist and parent consultant to:
There are several levels of services, which include: Early Identification Services: Through the Colorado Responds to Children with Special Needs program, any Broomfield child with a special need, or potential special need, is referred to the HCP program. The HCP care coordinator contacts the family to answer any questions and to assure them that children eligible for Early Intervention services are appropriately enrolled. Care Coordination Services: Includes facilitating collaboration and coordination among the family, the child's primary care provider/medical home, specialty care providers, Early Intervention services, schools and community resources. Ongoing Monitoring Services: Provides periodic developmental screening for children who do not qualify for services upon initial evaluation. HCP works in collaboration with Imagine! and Child Find.
