Public Health and Environment Division Healthy Communities Healthy Communities provides application assistance to pregnant women and families that may be eligible for Medicaid and/or the Child Health Plan Plus Program (CHP+). The program also assists those already receiving Medicaid and/or CHP+ in understanding their benefits and accessing medical, dental, vision, mental health and specialty services. We help pregnant women find providers for their prenatal care, helping to ensure a healthy pregnancy and a healthy baby. We encourage parents to take advantage of the preventive health care benefits for their children and help them to find providers for those services. By using preventive services, children will be less likely to end up in the emergency room for a condition that could have been treated in a doctor's office, or for treatment of a condition that could have been prevented. The Healthy Communities Program supports the" Medical Home" approach to health care. A medical home is not a place. It is a way to help you make sure all of your individual and/or family's health care needs are met. The medical home approach looks at the whole person, taking into consideration all medical, dental and mental/emotional health and community-based needs. Through a medical home approach to health care, your ideas, concerns, culture and values are respected. "Medical Home" is the 'new, old-fashioned care" - a partnership between you and your health care providers. Eligibility Health Communities is available for pregnant women of all ages and children, newborn through 20 years of age who are enrolled in Medicaid. Services
Health Screenings Health check-ups are important because they can help to identify health problems early so that treatment can be started early. Some health problems can become a lifetime illness if not treated early. Medicaid will pay for these health check-ups at no cost. If a problem is found that is not covered by Medicaid, the Family Health Coordinator will help find a program which will provide treatment at little or no cost.
